TZT14 Rebate

So I purchased a pair of TZT14's and am trying to send in my rebate offer.. One of the things necessary is the UPC code from the box.. Where is it? I cant find any UPC code on the box to cut out. I opened the box/boxes and there are sheets of paper inside with several of them having what looks to be UPC codes.. Should I use one of those?

The UPC code is on the outside of the shipping box. If you were not supplied this tag please contact the dealer from where you purchased the TZT.
